2019-2020 State Required Immunizations

All Incoming seniors will need to have a total of 2 Meningococcal Immunization done before the start of the school year.

All incoming 8th Graders will need to have a DTaP Booster and a Meningococcal Immunization done before the start of the school year.

Any junior high/high school student missing any Immunizations required by the state will need to have them done or scheduled to be done before beginning the school year. If scheduled, the appropriate form will need to be filled out by your doctor.

Students who are not up to date on the state required immunizations will not be allowed to begin the school year.

If you have any questions about your child’s immunizations record on file please contact me via email nurse@lamonte.k12.mo.us or by phone 660-347-5621 anytime. I am aware that many of the students have already had the proper immunizations but I do not have an updated record.

Please visit the Missouri Department of Health and Senior Services online for the information.

You can have your family doctor do them or the Pettis County Health Center does them for $20.00 by appointment. If you have any questions please contact Nurse Erin at 660-347-5621.
